With 15 million people living with long term conditions in the UK, a mechanism to support them to remember to take medication, monitor their symptoms and improve patient safety and outcomes is required.
Eastern AHSN has been working with uMotif to support the STPs in the East of England. uMotif is a low cost, scalable, engaging patient-centred digital platform, used to capture key outcomes, experiences and symptom data to help patients in self-care, GP practices to support patients safely, and Federations and Commissioners to understand care. uMotif has been supported by SBRI Healthcare since 2012. The platform has been used to support those with long term conditions and to support those who are completing a 6 week prescribed exercise programme with up to 40,000 licenses being made available for the Suffolk population.
Lessons learnt will be published July 2018 for this project. It is hoped that the practices involved will have seen a difference in those patients who have made use of the uMotif application and that patients feel more in control of their conditions as a result.
