Embracing Self-Care, One Year On is a one day event that is bringing together citizens, the NHS and industry to see what opportunities there are locally to accelerate improved self-management and shared decision making.
As well as exploring new opportunities for partnership working, we will also be showcasing fantastic projects already happening in our region, including:
- Active+ – a combination of peer support, education, advice and remote monitoring implemented alongside existing rehabilitation classes for cardiac and cancer in Huntingdonshire;
- Remote monitoring in care homes – in collaboration with Docobo, Cambridgeshire and Peterborough CCG have implemented remote monitoring in care homes for high risk patients; and,
- Lea Valley federation digital platform – a platform for the Hertfordshire GP federation which promotes self-care and a shift to digital for services that don’t require a physical appointment, reducing demand on primary care.
There will also be an opportunity to hear about Eastern AHSN’s latest work to enhance self-care in our region, with a call for bids to support innovative community projects to improve the lives of those with long term conditions and help the healthy maintain a positive lifestyle.
The event will take place during self-care week, an annual national awareness week that focusses on establishing support for self-care across communities, families and generations.
