Eastern AHSN is offering providers the opportunity to participate in a region wide rollout programme of PharmOutcomes.
This builds on work undertaken, in partnership with PrescQIPP and with Chief Pharmacists from across the region, to determine three collaborative projects to address Lord Carter’s recommendations for reducing unwarranted variation and improving productivity and efficiency.
One of these areas, digital pharmacy, aimed to explore opportunities for enhancing clinical pharmacy handover from acute providers to community pharmacists in order to support patients better adhere to medications, reducing potential adverse effects and unnecessary hospital visits.
Eastern AHSN will fund the first years’ licence fees for Trusts wishing to participate and will support coordination and implementation of the programme with regional project management support.
Trusts wishing to participate in the programme will be asked to sign a Memorandum of Understanding, along with instructions on expressing interest and next steps.
